Patient Registration

This is where it all starts. Below is a screenshot of the Patient Registration Form. Most entries are required except for email address, middle name, gender, civil status and referral.

If Payment method selected is HMO you are required to fill out the additional fields below.

Primary Card Holder:
Company (if paying through HMO):
Valid ID Type:
ID Number:

Once done with the registration, after clicking Register Patient, you will be taken to the Verify HMO page where, you will need to contact the Insurance company to verify the HMO details of the patient/client if Payment method selected is HMO.
